Overview
Advance care planning supports older people to make informed decisions about their future health care – before a time when they might not be able to speak for themselves. It helps ensure their values, beliefs and treatment preferences are known and respected if they become unwell or unable to communicate.
By having these conversations early, care providers and families can feel more confident delivering care that aligns with the older person’s wishes.

This culturally sensitive booklet was created to support First Nations communities in planning for future health care. With strong community input and thoughtful design, it helps older people – and those who support them – start important conversations about their values, wishes and end-of-life care.
The booklet guides users through choosing a Medical Treatment Decision Maker, recording personal beliefs, and completing a legal Advance Care Directive.
While designed with First Nations communities in mind, the clear, easy-to-follow format makes it a valuable resource for anyone wanting to plan ahead and ensure their voice is heard if they become too unwell to speak for themselves.
