Strengthening care management conversations & decision-making

  • Darebin Arts Centre 401 Bell Street Preston, VIC (map)

Confident conversations. Clear decisions. Safer care.

Sensitive conversations are part of everyday aged care practice. They can involve distress, reluctance, risk or changing needs. Staff need to communicate clearly and respectfully while supporting older people to stay in control of their choices.

This practical workshop builds the skills at the heart of care management under Support at Home and CHSP.

You’ll strengthen how you:

  • hold meaningful conversations

  • make decisions about risk and next steps

  • document clearly and consistently

  • support safe, person-led care

What you’ll learn

By the end of this workshop, you will be able to:

  • Hold structured, rights-based sensitive conversations that support choice and shared understanding.

  • Apply the Foundations for Supportive Conversations to complex care management discussions.

  • Strengthen judgement about risk and next steps, including escalation and referral.

  • Document clearly and consistently in ways that support safe care planning and review.

  • Respond professionally to distress, resistance or reluctance while supporting informed choice.

  • Communicate in ways that are culturally responsive and trauma-aware.

How you’ll learn

This is practical, scenario-based learning focused on real work situations.

You will:

  • work through 2 detailed case studies

  • practise skills in the worker role

  • receive structured feedback

  • connect learning directly to your day-to-day work

This approach builds confidence in practice, not just knowledge.
