The SSD Connect Alliance hosts online communities of practice for people working in CHSP and Support at Home-funded aged care.

Navigators Community of Practice

This Victorian-only online community of practice is for people who help older people access aged care services and supports.

We welcome people in a wide range of navigation roles, including Community Connectors, Healthy Ageing Officers, Care Finders, and Access and Support Workers.

What to expect

The community of practice provides a supportive forum where navigators can connect with colleagues, share experiences, exchange information and seek guidance on:

  • aged care reforms

  • wellbeing strategies

  • challenges and positive outcomes

  • work health and safety issues

  • de-identified scenarios.

Quality and Registration Renewal Community of Practice

This online community of practice is for people working to strengthen quality and prepare for registration renewal.

What to expect

The community of practice provides a supportive forum where participants can:

  • connect with colleagues facing similar challenges

  • explore solutions

  • exchange resources

  • learn from the experiences of other organisations

  • strengthen readiness for registration renewal.

Social Support Groups Community of Practice

This online community of practice is for people involved with planning, coordinating, leading and supporting social support groups, with a focus on high-quality, person-led and rights-based service delivery.

What to expect

The community of practice provides a supportive environment with opportunities to:

  • exchange ideas, resources and good practice examples

  • identify innovative approaches

  • problem-solve common challenges

  • explore changes arising from the aged care reforms

  • build connections with peers

  • contribute to shaping future topics.
