Community Connection & Service Navigation Workshop

Join us in person for a practical workshop. Build understanding of the My Aged Care assessment process and service navigation. Strengthen local referral pathways and connect with others doing similar work across the sector.

What we’ll cover

  • An update from the Department of Health, Disability and Ageing on My Aged Care assessment pathways and what they mean for frontline staff and volunteers

  • Practical service navigation scenarios and case studies, including common barriers and how to work through them

  • Map local referral pathways and clarify “who does what” across services

  • Identify service gaps and opportunities to strengthen collaboration

  • Formalise the Western Connector Group and agree next steps

Format

  • Short presentation

  • Panel discussion, including questions you submitted at registration.

  • Facilitated roundtable discussion based on real community connection and service navigation case studies.

  • Strengthen your approach to service navigation and referrals

  • Connect with other Community Connectors and Service Navigators and grow your local network
