Q: Can someone stay in a CHSP Social Support Group after moving to Support at Home?
A: Yes – if they were already attending one.
Participants who have transitioned from CHSP to Support at Home can continue to access their existing CHSP social support group on an ongoing basis.
This is delivered under the Social Support and Community Engagement service type. It’s designed to support continuity of social relationships.
Important:
This only applies if the participant was already attending the CHSP social support group before moving to Support at Home.
