Care and services plans: requirements and good practice

  • AEDT (VIC time) Online (map)

By 1 November 2026, all CHSP clients must have a care and services plan that meets the requirements of the Aged Care Act 2024 and Aged Care Rules 2025. Under Support at Home, a care plan must be in place before or on the day services start. Learn how to write plans that meet requirements and reflect what matters to older people.

Using a practical example, this session will explore person-centred planning, wellness and reablement, and how to turn assessments into meaningful goals and actions.

What you’ll learn

  • Understand the requirements under the Aged Care Act, Rules and Quality Standards, and identify the essential elements of a high-quality, person-centred plan.

  • Turn assessments into meaningful goals and actions, using the older person’s own words.

  • Include strengths-based planning, wellness and reablement in care and services plans.

  • Avoid common documentation pitfalls and know when and how to review and update plans.

  • Learn how to use the SSD Connect Alliance template and guidance tool in your service.
